Coverage is provided in the "EyeMed" Vision Care network.
To locate a provider in the network, go to the website www.eyemed.com, or ask your vision care provider if they are a member of EyeMed.
You may use any provider in the nation to obtain full benefits. There is no "network".
Claims are based on a multiple of the Medicare allowance, and the
vast majority of providers will accept this amount. In fact, your plan
guarantees that they will. If you incorrectly receive a "balance bill"
from your provider, contact Allied National's Elite Experience Team for assistance.

Most lab charges are covered under your plan and may be subject to your deductible and coinsurance.
If you incur lab charges through LabCorp OR Quest Diagnostics, they are covered at 100% with no deductible, therefore, tell your physician to use those labs, if possible.
